I give permission for my child to participate in the Voice of Kids: Future Journalist Training Program. I waive my claim on the Golden Key Cultural Center, Dixie Bloor Neighbourhood Centre, their staff or volunteers for any injury, loss or damage to my child’s person or property while involved in the above activity. I also authorize a representative of the Dixie Bloor Neighbourhood Centre or Golden Key Cultural Center to obtain emergency medical treatment if necessary. An attempt will be made to contact the Parent/Guardian first.

This form does not guarantee admission. Program admission based on child's writing ability and time of application. Since spots are limited, the program is on a first come first serve basis.
